Two federal awards under the Administration for Community Living’s Health at Home Challenge have positioned CATCH Greater Houston at the forefront of a national effort to stabilize care for dually eligible Medicare and Medicaid beneficiaries, providing an expanded framework for chronic disease management and post-hospital recovery across the region.

Local implementation involves a collaborative network including the Houston Health Department, Memorial Hermann, Harris Health, CommonSpirit Baylor St. Luke's, and UT Physicians. These partners provide core services such as care transitions, nutrition support, and fall prevention programs. A technological shift underpins this expansion: all consortium hubs are adopting the AthenaOne electronic medical record, marking a rare instance of unified data systems in community-based care. UTHealth Houston School of Public Health will lead an independent evaluation of the initiative's success in managing patient costs and improving clinical outcomes.
